What to say in email for letter recommendation? - letter of recommendation for hygiene school

I need a letter of recommendation (school is transmitted differently). I shall send you my math teacher for one too. I did it last year, in autumn 2007, so I do not think that you remember. What should I say in the e-mail? I do not want to sound arrogant or frightened away.

1. The email should sound something like this:

Dear Professor [name]:

Currently I am] [sec / junior] here [school, and I am thinking of transferring to another university. Since [current], which last year was [one of my favorite classes, one of my best marks / etc], I wondered if I would write a letter of recommendation. If so, I would like to give you all the information you need. But I also know that it is busy a time of year is when you can not help, then, that I like him, of course, I understand.

No e-mail him. Go see it. It'll give you a better chance of your memory. Moreover, it is really the only way that the teacher could not remember? Why not ask your advisor or professor this semester, or a smaller class where the teacher better known May.
